After completing this form, please read the steps for Undergraduate or Graduate applicants for further instructions.

The School of Music will contact you to set up an audition time once we receive your request. If you have not been contacted by the School of Music within 2 weeks of submitting this form please call 801-581-6762.

We are able to accomodate only a limited number of auditions after the scheduled audition dates, please contact the area head and Alyse Mourdock for further information.
